Job summary

BAYADA Home Health Care is seeking a compassionate Home Health Physical Therapist to serve patients in Nevada, MO and surrounding area. This full-time role offers a retention bonus and the chance to deliver care in patients' homes, reducing stress and burnout.

You will perform 1:1 direct care, develop individualized care plans, and work with an interdisciplinary team. Travel locally, with ongoing training and support, two-way communication, and growth opportunities.

Qualifications

• Active and current PT license.
• One year of clinical experience preferred.
• Previous home health care experience welcomed, but not required. We will train you!
• Ability to travel locally to cases as assigned.

Responsibilities

• Visit clients in their homes and support recovery after injury, illness, surgery, or hospitalization.
• Perform diagnostic tests and measurements of mobility, strength, balance, and safety.
• Develop and revise individualized PT care plans with manual therapy, gait training, and balance interventions.
• Educate clients and families in rehabilitative care and daily activities.
• Document observations and interventions on a state-of-the-art touch pad tablet.

We are recruiting for a compassionate Home Health Physical Therapist (PT) to reliably serve patients in Nevada, MO and surrounding area. This is a Full-Time position.

What you’ll do:

Visit a variety of clients in their own home, helping them recover from an injury, illness, surgery, or hospitalization.

Responsibilities for a PT - Physical Therapist include:

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
• Implement integumentary protective interventions to prevent wound formation and promote wound repair utilizing evidence-based dressings and topical agents.
• Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
• Continually assess and revise the Physical Therapy - PT care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.
• Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client’s health, safety, and independent living.
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.
