Dyersburg Health and Rehab in Dyersburg, TN is hiring a Physical Therapist to join our therapy team. You will plan, organize, develop and direct Physical Therapy services in accordance with standards, under direction of the Administrator, to ensure high-quality resident care.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in TN.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents per standards and procedures.
• Document and bill therapy services per CMS and company procedures.
• Evaluate residents to determine intervention needs and plan treatment.
• Develop short- and long-term treatment goals.
• Communicate evaluation findings and plans to residents, families, and team.
• Complete compliant documentation in NetHealth EMR.
• Maintain licensure through ongoing CE and professional development.
