Berlin Health & Rehabilitation Center in Barre, Vermont is seeking a full-time Physical Therapist (PT) to join our in-house rehabilitation team. The role offers $42–$52 per hour and a supportive, patient-centered environment where new graduates are welcome to apply.

You will evaluate residents, develop treatment plans, provide skilled PT interventions to improve mobility and function, document care, and collaborate with the interdisciplinary team to deliver high-quality rehabilitation services.

Formación

• Active Vermont Physical Therapy license (PT) is required.
• Experience in SNF/long-term care preferred.
• Strong clinical, communication, and interpersonal skills.
• Familiarity with electronic documentation systems (Net Health) preferred.

Responsabilidades

• Evaluate patients to determine PT needs and goals.
• Develop individualized treatment plans based on evaluations.
• Provide skilled PT interventions to improve mobility, strength, balance and independence.
• Administer prescribed treatments per plan of care.
• Monitor progress and adjust interventions as needed.
• Educate patients and caregivers on plans, exercises, and safety.
• Collaborate with nursing, therapy and medical staff.
• Document evaluations and treatment accurately and timely.
• Maintain compliance with regulations and facility policies.
• Participate in Rehabilitation Director meetings and duties.
