Job description

Role: Physical Therapist (PT) Field Supervisor | Las Vegas

  • Role: Field Supervisor (PT)
  • Comp: Salaried, exempt. $100,000 to $115,000 annualized
  • Mileage reimbursement for driving between visits, in addition to base
  • Schedule: Full-time, Monday through Friday
  • Location: Field-based, Las Vegas, NV (Durango office)
  • Reports to: Branch Clinical Manager

Adaptive Home Health is building a higher-acuity, patient-centered, skilled home health model across the US, and we launched our Las Vegas branch this year. Our ultimate mission is to dramatically improve patient access to home health care.

The Field Supervisor role is how we make that mission scale: you guide newly hired and transition clinicians through real-world home health practice, validate their clinical competency, and model the standard of care every patient deserves.

Who We Are

We build technology to better support our field clinicians - faster and more supported. Our clinicians are the customers of our engineering team. Our charting product is designed to reduce documentation burden and keep visits moving, and our scheduling product (launching soon) will make it easier to coordinate visits, reduce last-minute changes, and protect clinician time.

For Field Supervisors specifically, you'll have direct input into how those tools evolve through Adaptive focus groups - your frontline experience shapes the product roadmap.

If you have strong clinical judgment, love developing other clinicians, and want autonomy with real organizational impact, this role gives you a meaningful blend of patient care, teaching, and product influence.

What You Will Do
  • Time allocation: 75% field work (independently or with new joiners), 15% dedicated education time at the branch, 10% special projects (e.g., AI focus groups, improving education resources)
  • Precept and coach newly hired PTs and PTAs during patient visits, providing real‑time clinical guidance and feedback
  • Provide hands-on mentorship to new joiners, with particular focus on therapists with no prior home health experience
  • Evaluate and sign off on new joiners' clinical competency through structured observation during patient visits
  • Supervise PTAs, including required supervisory visits
  • Assist with skills lab training sessions and other structured onboarding activities
  • Administer and support end‑of‑orientation assessments for new hires
  • Serve as the primary point of contact for new joiners throughout orientation
  • Maintain a reduced but active patient caseload to stay current with clinical practice while dedicating meaningful time to precepting and teaching
  • Maintain quality standards, including a missed visit rate under 15%
  • Model best practices in OASIS documentation (including therapy-only starts of care), clinical decision‑making, and adherence to Medicare Conditions of Participation
  • Identify and elevate concerns about new joiner readiness or patient safety to the Branch Clinical Manager
  • Participate in AI charting product focus groups, contributing frontline clinical insight to Adaptive's tools and workflows
Why Adaptive
  • A multiplier role: You'll directly shape 20+ new clinicians per year, so your impact compounds in a way direct patient care alone can't.
  • Stable salaried comp: Predictable pay of $100,000 to $115,000, so your move doesn't come with comp uncertainty.
  • Real organizational influence: You'll join AI focus groups that drive product decisions affecting every field clinician.
  • Investment in your growth: We cover COS-C training and exam fees and provide an annual education fund.
  • Technology built for clinicians: Charting tools designed to reduce documentation burden.
  • Mission-driven work: We're here to expand access to high-quality home health care, and we want clinical leaders who take pride in raising the standard.
Requirements
  • Active, unrestricted Nevada PT license
  • Minimum 2 years of home health experience
  • Clean disciplinary record, with no adverse action in the last 12 months
  • No licensure encumbrances within the last 5 years
  • Current CPR/BLS certification
  • Current driver's license and comfort with field-based work
  • Strong clinical assessment skills and OASIS knowledge
  • Patient, encouraging communication style and the ability to give constructive feedback
  • Paraprofessionals (PTA, OTA, HHA) are not eligible
Nice-to-haves
  • Prior experience as a preceptor, clinical instructor, mentor, or educator
  • COS-C certification (or willingness to obtain it; we cover the cost)
  • Familiarity with Homecare Homebase (HCHB)
  • OASIS-E2 training or wound care certification
  • Experience training therapists who are new to home health
Clinical Focus Areas
  • Home health
  • OASIS-E2 accuracy, including therapy-only starts of care, recertifications, and discharges
  • Medicare Conditions of Participation
  • Functional, gait, balance, and fall-risk assessment
  • Therapy plans of care, goal setting, and visit frequency
  • PTA supervision and supervisory visits
  • Skills validation (transfers and mobility training, therapeutic exercise progression, DME and home safety assessment, wound care within PT scope)
Benefits
  • 401(k) with matching
  • Health, dental, and vision insurance
  • Paid time off
  • Continuing education credits and an annual education fund
  • Mileage reimbursement
  • Eligibility for our fleet program (company car), subject to waitlist
  • Adaptive branded scrubs, jackets, and vests
  • Referral program
