Job summary

EHS Home Health Care Services, Inc. is seeking a licensed Physical Therapist to provide patient-centered home health care across an assigned Illinois territory. You will evaluate and treat patients in their homes, coordinate with a multidisciplinary team, and document care to support quality and reimbursement.

The role involves travel within the territory, a full-time schedule, and rotating weekends/holidays, offering autonomy with strong team support and meaningful patient impact.

Qualifications

• Licensed physical therapist in Illinois or eligible for licensure.
• Minimum 1 year of clinical experience or strong clinical affiliation.
• Valid driver’s license and reliable transportation.

Responsibilities

• Deliver patient-centered home health care with high-quality assessments and treatments.

Physical Therapist – Home Health

Location: Calumet City, South Holland, Dolton, Riverdale, Harvey, Dixmoor, Phoenix, Posen, Burnham, Hegewisch, Thornton, Lansing, Glenwood, Lynwood

Schedule: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM
Rotating weekends required + 1 summer holiday & 1 winter holiday

Bonus: Up to $10,000 sign-on bonus for eligible external candidates (must be in role at time of payout; not available to current team members)

What You’ll Do

As a Home Health Physical Therapist, you will deliver high-quality, patient-centered care in a home setting, helping patients improve mobility, function, and quality of life. You’ll collaborate with an interdisciplinary team while working independently across your assigned territory.

Key Responsibilities

• Evaluate, develop, and implement individualized treatment plans based on patient needs and evidence-based practices
• Monitor patient progress and adjust care plans to achieve optimal outcomes
• Provide clear, timely, and compliant documentation to support care and reimbursement
• Educate patients and families on treatment plans, self-care, and safety practices
• Coordinate care with physicians, nurses, and other healthcare team members
• Participate in case conferences, team meetings, and quality improvement initiatives
• Support safe patient handling and promote fall and injury prevention

Requirements

Work Environment & Physical Demands

• Travel required within assigned service territory
• Work in patient homes and assisted living settings
• Ability to lift up to 35 lbs (with assistance/equipment as needed)
• Frequent standing, walking, bending, and assisting patients with mobility and transfers
