Job summary

Sigma Health Rehab is actively seeking a Full-time Physical Therapist (PT) to work in a Skilled Nursing Facility. This role offers competitive compensation, a comprehensive benefits package, generous PTO, CEU reimbursement, mentorship, and flexible schedules.

Qualified candidates hold a graduate degree from an accredited Physical Therapy program and a New York State PT license. Experience in SNF or geriatric care is preferred but new grads are encouraged to apply.

Responsibilities

• Evaluate residents’ conditions & develop appropriate physical rehabilitation plans
• Relieve resident’s pain using exercises, hands-on therapy & stretching
• Help residents through their tasks and monitor their performance
• Supervise the activities of physical therapy assistants
• Maintain resident confidence & protects facility by keeping information confidential
• Complies with federal, state, and local legal and professional requirements
• Maintains a cooperative relationship among healthcare team members
