Job summary

Lifepoint Rehabilitation is seeking a dedicated Physical Therapist for our Powers Health Rehabilitation Center in Crown Point, Indiana. This full-time position supports a team committed to patient-centered care.

Schedule is Monday–Friday, 8:00 a.m. to 4:30 p.m., with a $10,000 sign-on bonus. You will assess and treat adult patients, collaborate with nurses and therapy staff, and help patients regain function through evidence-based rehabilitation.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program.
• Current Physical Therapist licensure in Indiana.
• Inpatient rehab experience preferred.
• Current BLS/CPR certification.

Responsibilities

• Assesses patients' needs and initiates appropriate PT interventions.
• Works with the patient care team to maintain standards of practice.
• Assesses treatment impact and recommends changes to physicians.
• Performs assessments and timely QI scoring for adult and geriatric patients.
• Aims to reduce pain and enable participation in rehabilitation.

How you'll contribute

A Physical Therapist who excels in this role:

• Assumes responsibility and accountability for the care of those patients being treated by Physical Therapist Assistants and Therapy techs. May be assigned to orient staff and students and assume the duties and responsibilities of the supervisor in the absence of that individual
• Assesses patients' needs; identifies and initiates appropriate physical therapy interventions; works cooperatively with other patient team personnel in maintaining standards for professional physical therapy practice and interdisciplinary teamwork.
• Observes and evaluates treatment affect, accomplishment of short and long term goals. Recommends change to physician, if needed.
• Performs an appropriate assessment on each patient and performs reassessments, as per policy. Performs accurate and timely QI scoring. The population is adult and geriatric patients requiring an adult medical rehabilitation program and services.
• Able to assess patient pain interfering with optimal level of function of participation in rehabilitation; makes appropriate interventions to reduce pain; makes appropriate physician contact for intervention.

What we're looking for

Applicants should have a bachelor's degree from an accredited Physical Therapy school. Additional requirements include:

• Current licensure as a Physical Therapist in the state where the hospital is located
• Inpatient rehab experience preferred
• Current BLS/CPR Card
