Job summary

Lifepoint Rehabilitation in Crown Point, IN is seeking a dedicated Physical Therapist Assistant to implement treatment plans under supervision, monitor patient responses, and communicate progress to the supervising therapist.

You will document services, educate patients and families, assist with discharge planning, and help guide the selection of adaptive equipment and community support programs in a supportive, patient-centered environment.

Qualifications

• Associate degree in Physical Therapist Assistant required.
• Licensed Physical Therapist Assistant in IN required.
• Basic Life Support (BLS) certification required.

How you'll contribute

• Implements treatment plan developed by supervising therapist using appropriate modalities. Seeks consultation as necessary.
• Monitors patient's responses to the treatment plan. Regularly communicates patient progress and possible goal revisions to the physical therapist.
• Documents services provided and patient response/progress.
• Educates the patient and family/caregiver about patient deficits.
• Assists with patient discharge planning. Provides information regarding appropriate selection/use of adaptive equipment and community support programs.

What we're looking for

• Education: Associate's degree Required; Graduate of a Program in Discipline Required
• Licenses: Physical Therapy Assistant in the state of IN
• Certifications: Basic Life Support (BLS)
• Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
