Job summary

Madison Health and Rehab is seeking a Physical Therapist to join our Therapy Team to plan, organize, develop, and direct Physical Therapy services in accordance with applicable standards and regulations. A $10,000 sign-on bonus is offered for FT. Position supports flexible scheduling or PRN options.

Responsibilities include evaluating residents, developing treatment plans, documenting services, and communicating findings to families and the interdisciplinary team. Wisconsin licensure is required.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in Wisconsin.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents per standards and procedures.
• Document and bill therapies in NetHealth EMR.
• Evaluate and plan resident therapy programs with short and long-term goals.
