Qualifications

• Minimum of Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in Wisconsin is required.
• Ongoing continuing education to maintain licensure.
• Strong documentation and communication skills.

Responsibilities

• Provide physical therapy services to residents per standards and procedures.
• Adhere to documentation and billing procedures.
• Develop and implement individual treatment plans with short and long-term goals.
• Conduct regular resident screenings to determine intervention needs.
• Communicate evaluation findings and recommendations to the team and families.
