Summit Home Care and Hospice is searching for an experienced and motivated Physical Therapist (PT) to join our team. We’re committed to providing comprehensive healthcare services in the comfort of our patients' homes to positively impact quality of life by redefining the delivery of care.

Physical Therapist (PT)

• Full‑Time Hours: Monday - Friday; Standard Business Hours
• Work Setting: On the Road for Patient Visits & Training at the Local Office or Remotely
• Pay Structure: Pay per Visit, Salary or Salary Plus (varies depending on assigned territory)
• Estimated Salary: $80,000 - $110,000 per year + Sign‑On Bonus & Mileage

Responsibilities

• Provide physical therapy services according to a written physician’s plan of care and as defined in the state Physical Therapy Practice Act.
• Assess and evaluate therapeutic, rehabilitative/functional status.
• Develop, implement and direct physical therapy programs focused on rehabilitation.
• Evaluate the patient’s home environment.
• Assess patients for muscle strength, mobility, gait, ROM potential for rehab.
• Direct patients and families/caregivers in the use and care of therapeutic appliances.
• Determine priority needs for physical therapy.
• Report the patient’s reaction to treatment or changes in their condition to their physician.
• Initiate physical therapy program and instruct other personnel and/or family/caregiver members in certain phases of physical therapy.
• Instruct other personnel and/or family/caregiver the goals of the physical therapy program for the patient by participating in case conferences.
• May train the patient in the use of prosthetic devices.
• Identify patient and family/caregiver needs for other home health services and refer as necessary.
• Prepare and submit clinical and progress summaries based on the attainment of goals.
• Participate in discharge planning for the patient.
• Provide physical therapy consultation to home families/caregivers when indicated.
• Provide in‑service education programs for nursing organization personnel as needed.
• Participate in the peer consultation process.
• Supervise PTAs according to organization policy and state regulations.
• When therapy is the only skilled service, instruct, supervise and evaluate home health aide care every two weeks.
• Collaborate with other team personnel to achieve well‑rounded care.

Qualifications

• Active physical therapy license in the state of Ohio.
• Bachelor’s, master’s or doctorate in physical therapy from an accredited program.
• Current CPR certification.
• Recent TB test.
• Valid driver’s license with reliable transportation and active automobile insurance.

Preferred Experience

• Previous experience in physical therapy, rehabilitation, or related areas.
• Two years of appropriate experience as a physical therapist.
• Community/home health experience.
• Strong verbal and written communication, and organizational skills.
• Ability to handle physical workload.
• Ability to build rapport with patients.
• Strong leadership qualities.
