Job summary

Enhance Rehabilitation is seeking qualified Physical Therapists to join our team in a SNF setting. You will evaluate and treat patients following individualized plans of care, and collaborate with facility staff to deliver high-quality, patient-centered services.

You will document evaluations, progress notes and discharges, educate patients and families on outcomes, progress and expected timelines, and supervise Physical Therapist Assistants as needed.

• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re‑evaluations discharges and daily progress notes.
