Enhance Rehabilitation in Clifton, IL is seeking qualified Physical Therapists to join our dynamic SNF care team. You will evaluate and treat patients according to individualized plans of care, measure progress, and collaborate with facility staff to ensure high-quality outcomes.

In this role you will supervise Physical Therapist Assistants, document evaluations and progress notes, and communicate with patients and families about injury, treatment, and expected outcomes.

• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
