Job summary

Enhance Rehabilitation is actively seeking qualified Physical Therapists to join our team in SNF settings. You will evaluate and treat patients following an individualized plan of care, aiming to improve function and quality of life.

You will collaborate with facility staff, document evaluations and progress notes, and supervise Physical Therapist Assistants to ensure consistent, high-quality patient care. Excellent communication with patients and families is required.

Qualifications

• Qualified Physical Therapist with ability to evaluate and treat patients.
• Ability to develop and implement individualized plans of care.
• Strong collaboration with facility staff and clear communication with patients.
