Job summary

Synchrony Rehab is seeking a Physical Therapist to provide comprehensive physical therapy services in an interdisciplinary setting. You will assess patients, plan and implement individualized treatment programs, and educate patients and families on techniques to improve function.

Requirements include a degree in Physical Therapy from an accredited program and a current state PT license. Experience ranges from 0-1 years, with opportunities for professional growth in a supportive environment.

Responsibilities

• Conduct thorough assessments to evaluate patients’ physical conditions and rehabilitation needs.
• Develop and implement personalized treatment plans including therapeutic exercises, manual therapy, and modalities.
• Educate patients and families on exercise techniques, posture, body mechanics, and strategies to manage conditions.
• Maintain accurate documentation of progress, plans, and outcomes per facility policies.

Qualifications

The Physical Therapist (PT) is responsible for providing a full range of physical therapy services including assessment, treatment planning and therapeutic interventions in an interdisciplinary environment consistent with the position’s qualifications, professional practices and ethical standards.

Licenses and Certifications

Must have and maintain a current, valid state PT licensure or certification

Current valid CPR certification, preferred

Roles And Responsibilities

• Conduct thorough assessments to evaluate patients’ physical conditions, functional limitations, and rehabilitation needs.
• Develop and implement personalized treatment plans, including therapeutic exercises, manual therapy techniques, and modalities to improve patients’ physical function.
• Educate patients and their families on exercise techniques, posture, body mechanics, and strategies to manage their condition effectively.
• Maintain accurate and up-to-date documentation of patient progress, treatment plans, and outcomes in compliance with facility policies and regulatory requirements.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.
