Job summary

Synchrony Rehab at Warm Springs Medical Center in Warm Springs, GA is seeking a Physical Therapist to provide a full range of PT services, including assessment, treatment planning, and interventions within an interdisciplinary team.

You will evaluate patients, develop personalized plans with exercises and modalities, educate patients and families, and document progress in accordance with facility policies.

Qualifications

• Education: Degree in Physical Therapy from an accredited program; Experience: 0-1 years.
• Licenses and Certifications: Must have and maintain a current state PT licensure; CPR certification preferred.

Responsibilities

• Conduct thorough assessments to evaluate patients’ physical conditions and rehabilitation needs.
• Develop and implement personalized treatment plans, including exercises and modalities.
• Educate patients and families on exercises, posture, body mechanics and home programs.
• Maintain accurate documentation of progress and outcomes in compliance with policies.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.
