Job summary

Synchrony is seeking a Physical Therapist to provide a full range of PT services including assessment, treatment planning and therapeutic interventions in an interdisciplinary environment.

Role includes thorough assessments, personalized treatment plans, patient education, and precise documentation, with collaboration across the care team to ensure optimal patient outcomes.

Qualifications

• Education: Degree in Physical Therapy from an accredited program.
• Experience: 0-1 years.
• Licenses and Certifications: Must have and maintain a current state PT licensure or certification.
• CPR certification: Current valid CPR certification, preferred.

Responsibilities

• Conduct thorough assessments to evaluate patients’ physical conditions and rehabilitation needs.
• Develop and implement personalized treatment plans with exercises, manual therapy, and modalities.
• Educate patients and families on exercises, posture, and body mechanics.
• Maintain accurate documentation of progress, plans, and outcomes in compliance with policies.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.
