Job summary

Paramount Care Centers is seeking a Physical Therapist (PT) to coordinate and conduct physical therapy programs within our facility, promoting rehabilitation and independence for residents.

The role offers a pay rate of $48 per hour with a $60 per hour per diem option, plus benefits including medical, dental & vision, 401K, holiday pay, PTO, and overtime. We value compassionate, team-focused professionals who uphold high standards of care.

Qualifications

• Graduate of a physical therapy curriculum accredited jointly by the AMA and APTA.
• Certified by the APTA under its requirements.

Responsibilities

• Evaluates residents and develops physical therapy goals and treatment plans.
• Designs and fits adaptive devices and trains residents in their use.
• Selects modalities suited to individual needs and diagnoses.
• Consults with rehabilitation team to choose appropriate regimens.
• Instructs residents in mobility skills and evaluates progress.
• Adapts programs to resident needs and promotes safety.
• Attends care conferences and contributes to related meetings.
• Supervises supportive personnel per standards.
• Conducts training for PT techniques and objectives.
• Orders supplies and maintains equipment.
• Ensures quality of service in a compassionate environment.

POSITION SUMMARY:

The Physical Therapist organizes and conducts physical therapy programs for the facility to facilitate rehabilitation. He/she directs client participation in selected tasks to restore, reinforce, and enhance performance. In addition, the Physical Therapist facilitates the learning of those skills and functions essential for promoting independence and productivity, in order to diminish or correct pathology. He/she directs activities to promote and maintain health.

RESPONSIBLITIES/ ACCOUNTABILITIES:

1. 1 Evaluates residents, develops appropriate treatment goals, and methods, implements the physical therapy treatment plan, and completes all related documentation and record keeping regarding these services; services include application of therapeutic techniques in areas such as gait, ambulation, muscle strength, coordination, endurance, mobility, posture control, sensory awareness, sitting and standing skills, balance and orientation;
2. 2 Designs and fits adaptive devices, and trains residents in the use of adaptive devices including orthotics and prosthetics;
3. 3 Selects physical agent modalities suited to the individual’s needs, physical capacity, mobility activities and diagnosis;
4. 4 Consults with other members of the rehabilitation team to select the most appropriate therapeutic regimen consistent with the needs and capabilities of each resident;
5. 5 Instructs resident in the skills and techniques required for participation in mobility activities, and evaluates client progress, attitudes, and behavior as related to rehabilitative potential;
6. 6 Adapts program according to the needs of individual resident;
7. 7 Promotes a safe environment and enforces the elimination of fire and safety hazards;
8. 8 Attends and contributes to resident care, staffing conferences and other related meetings;
9. 9 Supervises supportive personnel in accordance with state and professional standards;
10. 10 Conducts training programs and participates in training medical, nursing and other personnel in physical therapy techniques and objectives;
11. 11 Orders supplies and equipment as necessary;
12. 12 Cleans, maintains, identifies, and reports the need for repair of equipment as necessary;
13. 13 Ensures that residents and families receive the highest quality of service in a caring and compassionate atmosphere that recognizes the individuals’ needs and rights.

SPECIFIC EDUCATIONAL/VOCATIONAL REQUIREMENTS:

Graduate of a physical therapy curriculum accredited jointly by the Council on Medical Education of the American Medical Association and the American Physical Therapy Association and certified by the American Physical Therapy Association under its requirements.

JOB SKILLS:
