Job summary

Maimonides Health Center in Virginia Beach, VA is seeking a Physical Therapist PT to deliver high-quality rehabilitation services to residents. Join a collaborative team at 6401 Auburn Dr, Virginia Beach, VA 23464.

Responsibilities include patient assessments, designing and administering treatment programs, documenting progress, and coordinating with the rehabilitation team. VA licensure is required; long-term care experience is preferred; competitive pay and benefits with a $500 referral bonus;

Qualifications

• Graduated from an accredited Physical Therapy college or university.
• Valid license and registration for the state of VA.
• Experience with long-term care preferred.

Responsibilities

• Assess referred patients requiring rehabilitation.
• Provide direct patient care and evaluate medical condition and functional capabilities.
• Develop and administer treatment programs with physician guidance.
• Document patient progress and educate patients and families.
• Collaborate with all disciplines for patient-centered care.
• Complete evaluations, progress notes, and discharge documentation.
• Perform duties assigned by the Director of Rehabilitation.

We are seeking a dedicated Physical Therapist PT to help us deliver outstanding rehabilitation services to our residents.

Your Role as a Physical Therapist PT:

At Maimonides Health Center, our Physical Therapist PT provides essential care to our residents by:

• Responsible for the assessment of referred patients that require rehabilitation services.

• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.

• Establish and administer a treatment program with specific goals determined according to the patient’s capacity and tolerance under the direction of the Physician.

• Set realistic and achievable goals for their patients

• Document and record the patients’ condition and educate patients and families in an appropriate physical therapy method.

• Evaluations, weekly progress notes, recertification and discharge documentations.

• Collaborates with all disciplines to provide best patient centered treatment and care.

• Perform all other duties assigned by Director of Rehabilitation

• Compliance with confidentiality, HIPPA and Federal Residents Rights.

What You Need:

• Graduated from an accredited Physical Therapy college or university

• Valid license and registration for the state of VA

• Experience with long-term care preferred
