Salary: $90,000 – $97,000 per year

Role Overview

We’re looking for passionate and experienced Physical Therapists (PTs) to join our growing team. This is a flexible role offering a mix of remote coordination and in-home care across the Austin area. You’ll work with patients to evaluate their needs, develop personalized treatment plans, and deliver high-quality therapy that supports recovery and independence at home.

Responsibilities

• Perform thorough patient assessments and evaluations
• Develop individualized treatment plans and update as needed
• Deliver skilled therapy interventions in patients’ homes
• Monitor patient progress and ensure compliance with physician orders
• Document all care accurately and in a timely manner
• Educate patients and caregivers, preparing them for discharge when appropriate
• Collaborate with the clinical team to ensure the best patient outcomes

Qualifications

• Active PT License in the state of Texas
• CPR Certification (Required)
• Home health experience (Required)
• Comfortable working with high-acuity patients
• Strong interpersonal and communication skills
• Reliable transportation and willingness to travel for home visits
• Self-motivated and capable of working independently

Preferred Background

• Experience in geriatrics, orthopedics, rehabilitation medicine, or sports medicine
• At least 1 years of home health experience
