Merit House is seeking a dedicated Physical Therapist to restore and enhance mobility and well‑being across outpatient, inpatient, or community settings. You will design personalized plans and deliver interventions through a collaborative, patient‑centered approach.

This role emphasizes evidence‑based therapies, meticulous documentation, and clear discharge planning to support seamless transitions and ongoing recovery for patients of all ages.

Formación

• Education: Doctor of Physical Therapy (DPT) or equivalent degree.
• Experience: Minimum 0 years; outpatient, inpatient or home health rehab preferred.
• Skills: physiology, anatomy, medical terminology, and physiotherapy techniques; strong EHR management and documentation.

Responsabilidades

• Assess patients’ physical conditions through evaluations to identify functional limitations and tailor treatment plans.
• Implement evidence-based therapies such as manual therapy, trigger point therapy, and exercise programs.
• Collaborate with physicians, nurses, therapists, and social workers to coordinate care and discharge strategies.
• Document patient progress in EHR/EMR with clinical standards.
• Manage discharge planning with detailed care plans for transitions home or to community care.
• Work with diverse patient populations, including geriatrics.
• Educate patients and families on goals, home exercises, and community resources.

Responsibilities

• Assess patients' physical conditions through comprehensive evaluations, including physiology and anatomy knowledge, to identify functional limitations and develop tailored treatment plans.

• Implement evidence-based therapeutic interventions such as manual therapy, trigger point therapy, and exercise programs to promote recovery and mobility.

• Collaborate effectively with healthcare professionals, including physicians, nurses, occupational therapists, and social workers, to coordinate care plans and discharge strategies.

• Document patient progress meticulously in electronic health records (EHR) systems while adhering to clinical documentation standards.

• Manage discharge planning processes by preparing detailed care plans that facilitate seamless transitions from inpatient settings to home or community care environments.

• Work with diverse patient populations, including those in geriatrics.

• Educate patients and their families on treatment goals, home exercise routines and community resources to support ongoing recovery outside clinical settings.

Requirements

• Education: Doctor of Physical Therapy (DPT) or equivalent degree.

• Experience: Minimum 0 years of experience; experience in outpatient clinic settings, inpatient hospital environments, or home health services with a focus on patient rehabilitation is preferred.

• Skills: Extensive knowledge of physiology, anatomy, medical terminology, and physiotherapy techniques; strong skills in clinical documentation standards and electronic health records (EHR/EMR systems) management; ability to develop comprehensive treatment plans; excellent client communication skills.
