Vista Manor Healthcare & Rehabilitation in Titusville, FL is seeking a dedicated Physical Therapist (PT) to join our on-site team. This full-time role focuses on developing and implementing individualized PT plans for residents, with collaboration across departments to ensure seamless care.

You will evaluate patients, provide treatments to restore function and reduce pain, and mentor staff while maintaining thorough documentation and adherence to facility guidelines.

Qualifications

• Bachelor’s Degree in Physical Therapy from an accredited institution.
• Active Physical Therapy license in Florida.
• Experience in skilled healthcare settings is required.

Responsibilities

• Develop, implement, and evaluate individualized physical therapy programs per long-term care guidelines.
• Collaborate with administration, medical, nursing, and other departments to coordinate therapy services.
• Maintain high PT standards and ensure quality care for all residents.
• Oversee and manage the physical therapy department, including staffing and recruitment.
• Provide training and mentorship to new personnel.
• Conduct patient evaluations, tests, and measurements to inform treatment plans.
• Administer PT treatments to restore function, relieve pain, and prevent disability.
• Document patient evaluations, care plans, progress notes, and reports accurately and timely.
