• Deliver a variety of therapeutic interventions such as therapeutic exercises, trigger point therapy, manual therapy, and modalities tailored to patient needs.
• Collaborate closely with healthcare professionals—including physicians, nurses, occupational therapists—to coordinate care and discharge planning.
• Monitor patient progress regularly; adjust treatment plans accordingly to maximize recovery and rehabilitation outcomes.
• Maintain accurate electronic health records (EHR) and electronic medical records (EMR) systems for all patient interactions and documentation.
• Educate patients and their families on treatment procedures, home exercises, safety measures, and community resources to support ongoing recovery.
• Participate in team meetings to review treatment strategies, share insights on patient progress, and contribute to multidisciplinary care planning.

Requirements:

• Proven experience practicing in a physical therapy role within outpatient clinics or inpatient hospital settings.
• Demonstrated expertise in discharge planning, treatment plan development, and patient rehabilitation strategies.
• Hands-on experience working with geriatric patients.
• Familiarity with clinical documentation standards and medical terminology necessary for accurate record keeping.
• Experience managing electronic health records (EHR) systems or electronic medical records (EMR) platforms efficiently.
• Strong knowledge of physiology and anatomy relevant to physiotherapy practices across diverse patient populations.
• Ability to collaborate effectively with healthcare professionals across disciplines while maintaining excellent client communication skills.
