Location: Lufkin, TX and Multiple Locations, Texas Position Type: Full-Time, Part-Time, or PRN

Job Summary

We are seeking a compassionate, dedicated, and licensed Physical Therapist (PT) to join our Home Health & Hospice team. The Physical Therapist is responsible for evaluating patients' physical conditions, developing individualized treatment plans, and providing therapeutic interventions to improve mobility, reduce pain, and maximize functional independence. The PT will work collaboratively with physicians, nurses, and interdisciplinary team members to provide high-quality patient-centered care in the patient's home.

Essential Job Duties and Responsibilities
  • Perform comprehensive physical therapy evaluations and assessments.
  • Develop individualized treatment plans based on physician orders and patient needs.
  • Provide skilled physical therapy services in accordance with state and federal regulations.
  • Evaluate patients' functional abilities, including mobility, strength, balance, coordination, and endurance.
  • Establish measurable treatment goals and revise plans of care as needed.
  • Educate patients and caregivers regarding treatment plans, exercises, safety measures, and disease management.
  • Provide therapeutic exercises, gait training, balance training, transfer training, and pain management techniques.
  • Assess home environments and recommend adaptive equipment or modifications to improve patient safety.
  • Complete all required documentation accurately and timely in the Electronic Medical Record (EMR) system.
  • Communicate effectively with physicians and interdisciplinary team members regarding patient progress and changes in condition.
  • Participate in case conferences, care coordination meetings, and interdisciplinary team meetings when required.
  • Maintain compliance with Medicare Conditions of Participation, HIPAA regulations, agency policies, and professional standards.
  • Supervise Physical Therapist Assistants (PTAs) when applicable and provide clinical guidance in accordance with state regulations.
  • Participate in orientation programs, competency assessments, and continuing education activities.
  • Perform other duties as assigned by the Clinical Supervisor or Director of Patient Care Services.
Qualifications
Education
  • Graduate of an accredited Physical Therapy program approved by the Commission on Accreditation in Physical Therapy Education (CAPTE).
  • Bachelor's, Master's, or Doctor of Physical Therapy (DPT) degree required.
Licensure and Certifications
  • Current and unrestricted Physical Therapist (PT) license in the State of Texas.
  • Current CPR certification required.
  • Valid Texas Driver's License.
  • Current automobile insurance and reliable transportation required.
Experience
  • Minimum one (1) year of clinical experience as a Physical Therapist preferred.
  • Home Health and/or Hospice experience preferred.
  • Experience working with geriatric patients is highly desirable.
  • Knowledge of Medicare regulations and documentation requirements is preferred.
Knowledge, Skills, and Abilities
  • Strong clinical assessment and treatment planning skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and manage time effectively.
  • Knowledge of Home Health and Hospice regulations and standards of practice.
  • Ability to educate patients and caregivers effectively.
  • Strong documentation and computer skills.
  • Ability to establish positive working relationships with patients, families, physicians, and interdisciplinary team members.
  • Demonstrates compassion, professionalism, and cultural sensitivity when providing patient care.
Physical Requirements
  • Ability to stand, walk, bend, lift, and transfer patients as necessary.
  • Ability to frequently lift up to 50 pounds with or without reasonable accommodation.
  • Ability to travel to patients' homes within assigned service areas.
  • Ability to perform patient assessments and therapeutic interventions in various home environments.
