Job summary

Honey Grove Nursing Center is seeking a full-time Physical Therapist (PT) to join our in-house rehab team. You will assess residents, develop and implement individualized treatment plans, and collaborate with an interdisciplinary team to improve mobility and quality of life.

The ideal candidate holds an active Texas PT license, has SNF or long-term care experience, and is familiar with Net Health. New graduates are welcome to apply; competitive benefits and strong corporate support are offered.

Qualifications

• Active and valid Texas Physical Therapist license.
• In good standing with regulatory agencies and licensing boards.
• SNF or Long-term Care experience preferred.
• Current knowledge of physical therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.
• Ability to manage patients with varying personalities and needs.
• Strong communication, collaboration, and clinical decision-making skills.

Responsibilities

• Consult with patients to assess their physical condition and functional limitations.
• Evaluate and interpret patient assessments and test results.
• Develop individualized treatment plans using a variety of therapy techniques.
• Create personalized rehabilitation programs tailored to each patient’s goals.
• Administer medically prescribed physical therapy treatments to relieve pain, restore function, and promote healing.
• Ensure all treatments are delivered in accordance with the established plan of care.
• Monitor patient response to treatment interventions and modify approaches as clinically appropriate in collaboration with the interdisciplinary team.
• Provide clinical support and education to patients, family members, and caregivers.
• Collaborate with interdisciplinary team members to ensure high-quality patient care.
• Complete documentation thoroughly and timely in compliance with company policies and state/federal regulations.
• Comply with all applicable rules, regulations, and procedures.
• Attend required meetings as designated by the Director of Rehab
