Job summary

Wellbrooke of South Bend in Indiana is seeking a licensed Physical Therapist to provide a full range of PT services within an interdisciplinary team. The role includes assessment, treatment planning, and therapeutic interventions while upholding professional and ethical standards.

You will conduct thorough evaluations, develop individualized treatment plans, educate patients and families on exercises and body mechanics, and document progress to ensure regulatory compliance.

Qualifications

• Must have current state PT licensure.
• CPR certification is preferred.
• Education: Degree in Physical Therapy from an accredited program.

Responsibilities

• Conduct thorough assessments to evaluate patients’ physical conditions and rehabilitation needs.
• Develop and implement personalized treatment plans with exercises, manual therapy, and modalities.
• Educate patients and families on exercises, posture, body mechanics, and self-management strategies.
• Maintain accurate documentation of patient progress, plans, and outcomes per facility policies.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.

Position Overview

The Physical Therapist (PT) is responsible for providing a full range of physical therapy services including assessment, treatment planning, and therapeutic interventions in an interdisciplinary environment consistent with the position’s qualifications, professional practices, and ethical standards.

Licenses and Certifications

Must have and maintain a current, valid state PT licensure or certification.

Current valid CPR certification is preferred.

Roles and Responsibilities

• Conduct thorough assessments to evaluate patients’ physical conditions, functional limitations, and rehabilitation needs.
• Develop and implement personalized treatment plans, including therapeutic exercises, manual therapy techniques, and modalities to improve patients’ physical function.
• Educate patients and their families on exercise techniques, posture, body mechanics, and strategies to manage their condition effectively.
• Maintain accurate and up-to‑date documentation of patient progress, treatment plans, and outcomes in compliance with facility policies and regulatory requirements.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.
