Job summary

Personal Touch Home Care seeks a dedicated Physical Therapist to deliver home-based therapy in Somerset, KY. You will assess patients, develop care plans, and ensure environments support rehabilitation goals, following physician orders.

Qualifications

• Licensed to practice as a physical therapist in Kentucky.
• Degree from a PT program approved by APTA/AMA committees.
• Preferred: at least 1 year of home health experience.
• Strong verbal and written communication and organizational skills.

Responsibilities

• Assess the therapeutic, rehabilitative, and functional status of patients.
• Participate in developing comprehensive care plans tailored to each patient.
• Evaluate the home environment to support rehabilitation goals.
• Assess muscle strength, mobility, gait, ROM, and rehabilitation potential.
• Instruct patients/caregivers in use and care of therapeutic appliances.

Job Details Overview:

As a Physical Therapist with Personal Touch Home Care, you'll play a crucial role in providing direct physical therapy services based on the physician's plan of care. Your responsibilities will include:

• Assessing and evaluating the therapeutic, rehabilitative, and functional status of patients.
• Participating in the development of comprehensive care plans tailored to each patient's needs.
• Evaluating the home environment to ensure it supports the patient's rehabilitation goals.
• Assessing muscle strength, mobility, gait, range of motion, and the patient's potential for rehabilitation.
• Instructing patients/caregivers in the use and care of therapeutic appliances to support their rehabilitation journey.

Qualifications

• Licensed to practice a physical therapist within the State
• Possesses a degree from a baccalaureate or master’s program in physical therapy approved by The American Physical Therapy Association, or The Committee on Allied Health Education and Accreditation of the American Medical Association or The Council on Medical Education of the American Medical Association.
• Prefer one year of appropriate experience as a physical therapist. Home health experience is preferred.
• Demonstrates good verbal and written communication and organization skills.
