Job summary

Personal Touch Home Care of Kentucky Inc is seeking a Physical Therapist to provide direct physical therapy services in home care, following the physician’s plan of care. You will assess patients, participate in care planning, and ensure home environments support rehabilitation goals.

The role requires a Kentucky PT license, a PT degree, and strong communication and organizational skills. Home health experience is preferred. Pay is competitive and full-time benefits are provided.

Qualifications

• Requires Kentucky licensure to practice physical therapy.
• Degree from a PT program approved by the American Physical Therapy Association or equivalent.
• Prefer one year of home health experience.
• Demonstrates good verbal and written communication and organization skills.

Responsibilities

• Assess and evaluate the therapeutic, rehabilitative, and functional status of patients.
• Participate in development of comprehensive care plans tailored to each patient’s needs.
• Evaluate the home environment to support rehabilitation goals.
• Assess muscle strength, mobility, gait, range of motion, and rehabilitation potential.
• Instruct patients and caregivers in use and care of therapeutic appliances.

Job Details Overview:

As a Physical Therapist with Personal Touch Home Care, you'll play a crucial role in providing direct physical therapy services based on the physician's plan of care. Your responsibilities will include:

• Assessing and evaluating the therapeutic, rehabilitative, and functional status of patients.

• Participating in the development of comprehensive care plans tailored to each patient's needs.

• Evaluating the home environment to ensure it supports the patient's rehabilitation goals.

• Assessing muscle strength, mobility, gait, range of motion, and the patient's potential for rehabilitation.

• Instructing patients/caregivers in the use and care of therapeutic appliances to support their rehabilitation journey.

Qualifications

• Licensed to practice a physical therapist within the State

• Possesses a degree from a baccalaureate or master’s program in physical therapy approved by The American Physical Therapy Association, or The Committee on Allied Health Education and Accreditation of the American Medical Association or The Council on Medical Education of the American Medical Association.

• Prefer one year of appropriate experience as a physical therapist. Home health experience is preferred.

• Demonstrates good verbal and written communication and organization skills.

Pay Type: Full-time

Pay: $110,000-$115,000/ annually
