Job summary

Choice Health at Home in Sarasota, FL is seeking a licensed Physical Therapist to provide in‑home therapy services. You will evaluate patients, create individualized treatment plans, and deliver therapeutic programs to restore function and mobility.

The role requires a valid FL PT license, CPR certification, and a valid driver's license. Home health experience is preferred, and a PRN schedule may apply to support patient needs.

Qualifications

• Graduate of an accredited physical therapy program and licensure exam.
• Current, valid Physical Therapist license.
• Current CPR certification.
• Valid driver’s license and auto insurance.

Responsibilities

• Perform and document comprehensive initial evaluations to identify patients’ needs and establish individualized treatment plans.
• Develop and deliver therapeutic programs to restore function, improve mobility, relieve pain, and prevent physical decline.
• Monitor patient progress and adjust treatment plans to achieve the best outcomes.
• Educate patients and families on exercises, safety, and strategies for maintaining independence.
• Collaborate with patients, families, and other healthcare professionals to ensure continuity of care.
• Follow infection control practices and universal precautions.
• Complete accurate documentation within company-established timelines.

What You’ll Do
