Preferred Care Home Health is seeking a skilled Physical Therapist (PT) for Home Health PRN in Florida. You will evaluate patients, develop personalized treatment plans, and deliver therapy in patients' homes, adapting to physician orders and agency policy.

You'll track progress, educate patients and caregivers, document findings accurately, and collaborate with the care team. PRN schedule offers flexibility and meaningful, patient-centered care.

Qualificações

• Degree from an accredited Physical Therapy program.
• 1+ year of physical therapy experience.
• Home health experience a plus, not required.
• Current CPR certification.
• Valid driver's license, reliable transportation, and auto insurance.
• Strong clinical judgment, communication, and documentation skills.

Responsabilidades

• Perform comprehensive PT evaluations and build individualized treatment plans.
• Deliver skilled physical therapy in line with physician orders and agency policy.
• Track patient progress and adjust care plans as needed.
• Teach patients and caregivers safe techniques, exercises, and equipment use for lasting results.
• Document visits and clinical findings accurately and on time.
• Flag changes in patient condition to physicians and the care team.
• Provide clinical supervision to PTAs.
• Take part in case conferences, care planning, and in-service education.
