We are seeking a dedicated and compassionate Physical Therapist to join our team in a skilled nursing facility.
RESPONSIBILITIES:
Conduct comprehensive evaluations of patients to identify physical, functional, and rehabilitation needs.
Develop and implement individualized treatment plans in collaboration with the interdisciplinary team.
Monitored and documented patient progress and adjusted treatment plans as necessary.
Educate patients and caregivers on exercises and techniques to promote recovery and prevent future injury.
Provide hands-on therapy, including exercises, manual therapy, and modalities as appropriate.
Ensure compliance with regulatory standards and company policies in all aspects of care delivery.
Participate in clinical meetings and contribute to a culture of continuous improvement.
QUALIFICATIONS:
New passer/graduates are welcome
With Skilled Nursing or Long-Term Care, experience is a plus
Previous experience as a Physical Therapist (PT) is a plus
Physical Therapy Degree from an accredited program
State licensure or eligibility for licensure as a physical therapist
Is in good standing with all regulatory agencies and licensing boards as required
Interest in working with the geriatric population
Ability to work effectively both independently and within a team
