A registered professional Physical Therapist (PRN) who provides coordinated care to patients of all age groups within the hospice setting on an as-needed basis. The Physical Therapist plans, implements, and evaluates patient care plans to maintain and improve patient well-being. This role involves planning, organizing, and conducting physical therapy treatment programs based on medical referrals and patient evaluations. The Physical Therapist collaborates with the Interdisciplinary Group/Team (IDT) to ensure comprehensive patient care.

Job Qualifications:Licensure:- Current state license as a Physical Therapist, OR
- A multi-state license issued by a Physical Therapist Licensure Compact (PTLC) member state if practicing within the PTLC.
Experience:- experience as a Physical Therapist in an acute care or rehabilitation setting.
Skills:- Strong clinical therapy skills as defined by accepted standards of practice.
- Effective interpersonal and communication skills.
- Knowledge of durable medical equipment and its proper use.
Transportation:- Must have reliable transportation and valid auto liability insurance.
Environmental and Working Conditions:- Work is performed in healthcare facilities and patient homes under various conditions.
- Possible exposure to blood, body fluids, and infectious diseases.
- Requires flexible scheduling, including availability on short notice for PRN coverage.
- Local travel required with potential exposure to unpleasant weather while traveling between patient locations.
Physical and Mental Effort:- Requires heavy lifting, bending, pulling, pushing, and prolonged standing.
- Frequent walking and manual dexterity required.
- Must be able to work under stress, balancing patient needs and deadlines.
- Must be comfortable working with terminally ill patients and their families, providing emotional and physical support.
- Requires strong eye-hand coordination and fine motor skills.
Essential Functions:Evaluation & Treatment:- Perform comprehensive clinical assessments and identify patient needs.
- Document, prepare, and complete a Plan of Care (POC), progress notes, phone orders, and clinical records on an ongoing basis.
- Revise the POC as needed, based on evaluation and patient progress.
- Plan and provide appropriate treatment for neuromuscular, neurological, and orthopedic conditions based on medical referrals.
Patient Care & Safety:- Provide safe and effective physical therapy interventions based on patient-specific needs.
- Assist and instruct patients/families in transfer techniques and the proper use of mobility equipment (e.g., walkers, wheelchairs, canes).
- Ensure that therapy interventions align with patient goals and hospice philosophy.
- Apply infection control protocols and standard precautions to prevent contamination and transmission of diseases.
- Provide education and training to caregivers and family members regarding therapy techniques and patient mobility.
Collaboration & Compliance:- Work closely with the Interdisciplinary Group (IDT) to ensure coordinated patient care.
- Accept PRN assignments that match education and competency to ensure quality care.
- Meet continuing education requirements set by Harmony Hospice Care and the licensing board.
- Demonstrate professional behavior in adhering to hospice policies and regulations.
- Use effective interpersonal relations and communication skills to support patients, families, and healthcare teams.
