Job summary

Highland Chateau Health + Rehabilitation seeks a PRN In-House Physical Therapist (PT) to join our rehabilitation team. The PT will perform patient assessments, develop treatment plans, and deliver therapy under physician direction.

Eligible candidates hold an MN license, have experience in long-term or geriatric care, and are proficient with EMR documentation while maintaining HIPAA compliance and exceptional patient service.

Qualifications

• Graduated from an accredited Physical Therapy college program or university.
• Valid MN license/registration to practice as a PT.
• Experience with long-term care and/or geriatric populations preferred.
• Proficient with EMR documentation and record-keeping.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Develop and administer treatment plans with goals per patient capacity.
• Document patient condition and educate patients and families about the plan of care.

Highland Chateau Health + Rehabilitation is hiring for a PRN In-House Physical Therapist (PT) candidates to become a part of our team!

Responsibilities Include:

• The Physical Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions, and potential for rehabilitation.
• The Physical Therapist will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician.
• The Physical Therapist will set realistic and achievable goals for their patients, document and record the patient's condition and educate patients and families with regard to the plan of care.

Qualifications/Requirements:

• Graduated from an accredited Physical Therapy college program or university
• Valid professional license and registration for the state of MN
• Experience with long-term care and/or geriatric care is preferred but not limited to
• Well-versed with EMR documentation Evaluations, weekly progress notes, recertification, and discharge documentation
• Collaborates with all disciplines to provide the best patient-centered treatment and care
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Adhere to scheduled hours and demonstrate professionalism at all times
• Perform all other duties assigned by the Director of Rehabilitation Compliance with confidentiality & HIPAA
