Southview Acres Health Care Center is seeking a Full Time or PRN Physical Therapist to join our team in Minnesota. The role involves evaluating referred residents, developing individualized treatment plans, and documenting progress under the physician's direction.

You will collaborate with the rehabilitation team to deliver patient-centered care and help residents maintain function. Applicants must have graduated from an accredited PT program and hold a Minnesota license.

Formación

• Graduated from an accredited Physical Therapy program or university.
• Valid professional license and registration for MN.
• Experience with long-term care or geriatric care preferred.
• Proficient with EMR documentation including evaluations, progress notes, recertification, and discharge details.
• Collaborates across disciplines to provide patient-centered care.
• Strong patient service mindset and clinical excellence.
• Adheres to schedule and maintains professionalism; HIPAA confidentiality.

Responsabilidades

• Assess referred patients requiring rehabilitation and provide direct patient care to evaluate condition and functional abilities.
• Establish and administer a treatment program with goals aligned to patient capacity under physician direction.
• Set realistic goals, document patient status, and educate patients and families on the plan of care.

Responsibilities Include:

• The Physical Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.

• The Physical Therapist will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician.

• The Physical Therapist will set realistic and achievable goals for their patients, document and record the patient's condition and educate patients and families with regards to the plan of care.

Qualifications/Requirements:

• Graduated from an accredited Physical Therapy college program or university

• Valid professional license and registration for state of MN

• Experience with long-term care and/or geriatric care is preferred but not limited to

• Well versed with EMR documentation Evaluations, weekly progress notes, recertification and discharge documentations

• Collaborates with all disciplines to provide best patient centered treatment and care

• Exceptional patient service skills and ability to demonstrate clinical excellence

• Adhere to scheduled hours and demonstrate professionalism at all times

• Perform all other duties assigned by Director of Rehabilitation Compliance with confidentiality & HIPAA
