AdviniaCare Seashore Pointe is seeking a compassionate Physical Therapist to join our interdisciplinary team in Provincetown, MA. You will design and implement personalized treatment plans to promote independence, safety and quality of life for residents.

As a Licensed Physical Therapist, you will work with the rehabilitation team to assess needs, educate residents and families, and monitor progress while maintaining clear records.

Responsibilities

• Assess resident needs and develop individualized treatment plans to achieve goals
• Implement and modify treatment plans to address physical, cognitive, and emotional needs
