Job summary

Interim HealthCare in Pratt, Kansas, is seeking a Home Health Physical Therapist (PT) to deliver compassionate, home-based therapy. You will assess mobility, design plans of care, and work with a dedicated home health team to help patients regain strength in familiar surroundings.

This role supports one-on-one patient care with autonomy and ongoing education, plus opportunities for online training and CEUs.

Qualifications

• Graduate of an accredited Physical Therapist Program and active Kansas PT license.
• CPR certification.
• Good clinical judgement, strong interpersonal skills, dexterity and compassion.
• Ability to work in home health setting with mobility needs.

Responsibilities

• Provide home-based physical therapy to patients who have lost mobility, strength and functionality due to illness or injury.
• Work as part of a home health team which may include an RN, LPN, CNA, HHA, OT and SLP, focused on the patient's plan of care and goals.
• Observe movements, conduct assessments, document progress and modify care plan as needed.
• Assist patient with exercises and hands‑on therapy to manage pain and increase mobility.
• Educate patients on their plan of care, established goals, exercises and self-care.
• Coach family on the plan of care, therapy goals and safely assisting with exercises.
