Job summary

Interim HealthCare Inc. in Portsmouth, NH is seeking a Home Health Physical Therapist to deliver personalized care in patients' homes, helping them regain mobility and independence.

You will conduct evaluations, develop treatment plans, provide hands-on therapy, and coordinate with nurses and therapists to support safe, effective recovery while enjoying flexible scheduling and professional growth. Qualifications

• Graduate of an accredited Physical Therapy program with entry-level readiness.
• Active NH PT license and CPR certification required.
• Experience enabling independent clinical judgment in home health preferred.

Responsibilities

• Evaluate patients and create individualized treatment plans.
• Restore strength, mobility, balance and functional independence.
• Provide hands-on therapy and therapeutic exercise.
• Monitor progress and adjust care plans as needed.
• Collaborate with nurses, OT, speech therapists, aides and clinicians.
• Educate patients and families on exercises and safety strategies.
• Complete accurate clinical documentation in a timely manner.

As a Home Health Physical Therapist, you’ll help patients recover safely and confidently in their own homes by:

• Evaluating patients and developing individualized treatment plans
• Restoring strength, mobility, balance, and functional independence
• Providing hands‑on therapy and therapeutic exercise
• Monitoring progress and adjusting care plans as needed
• Collaborating with nurses, occupational therapists, speech therapists, home health aides, and other clinicians to coordinate care
• Educating patients and family members on exercises, safety strategies, and ongoing recovery
• Completing accurate and timely clinical documentation

What We’re Looking For

• Graduate of an accredited Physical Therapy program
• Active New Hampshire Physical Therapist license
• Minimum of one year of PT experience (home health experience preferred)
• Current CPR certification
• Knowledge of state and federal home health regulations
• Excellent clinical judgment, communication, and patient education skills
• Compassionate, organized, and committed to delivering exceptional patient care
