Job summary

Signature Rehab is seeking a Physical Therapist to join our team in McCreary County, KY. The role is PRN with consistent hours, focusing on delivering high-quality rehabilitation services to residents and outpatient clients within Signature HealthCARE facilities.

Qualified candidates hold a Bachelor’s in Physical Therapy and a current PT license for the state. We offer a comprehensive benefits package and a supportive, mission-driven work environment.

• Graduate of accredited program in physical therapy.
• A valid Physical Therapy license in the state where services are rendered.

Responsibilities

• Under Signature Rehab’s integrated care model, the rehabilitative therapist works closely with other clinical caregivers to ensure residents and patients make a full recovery and a timely return to activities.
• Support care plans and document progress in patient records.
• Provide patient education and family communication as part of the therapy process.

• Graduate of accredited program of physical therapy, culminating in a minimum of a Bachelor's of Science Degree
• A Valid Physical Therapy license in the state where services are rendered.
