Cliveden Nursing & Rehab Center in Philadelphia, PA is seeking a full-time Physical Therapist to join our in-house rehab team and help patients regain function with evidence-based care.

We welcome new graduates and offer mentorship, supportive colleagues, and a comprehensive benefits package. Pennsylvania PT license required or eligibility to obtain, with a collaborative, patient-centered work culture.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license in Pennsylvania (or eligibility to obtain).
• Strong communication and interpersonal skills.
• Passion for providing exceptional patient care.
• New graduates are encouraged to apply.

Responsibilities

• Evaluate residents and patients and develop individualized treatment plans
• Provide skilled physical therapy services in accordance with physician orders and established plans of care
• Educate patients, families, and caregivers regarding treatment goals and therapeutic interventions
• Monitor patient progress and modify treatment plans as appropriate
• Complete timely and accurate documentation in accordance with company policies and state and federal regulations
• Communicate patient status and progress with physicians, nursing staff, and interdisciplinary team members
• Assist with scheduling and accurately complete daily treatment charges
