Job summary

Constant Care Health Services is currently seeking highly trained Physical Therapists (PT) for our 5 Star Skilled Nursing Facilities throughout the Bronx. This part-time, weekday and weekend role offers per diem arrangements, W-2 status with full benefits, and ample training and support to ensure your success.

Qualified candidates hold a NYS PT License, demonstrate strong communication skills, and are available to work weekends when needed.

Qualifications

• NYS PT License is required.
• Weekend availability required.

Responsibilities

• Assess client needs on admission for the service as appropriate.
• Implement and revise a medical plan of care as necessary.
• Initiate preventive, therapeutic and rehabilitative techniques.
• Educate the client and family regarding treatment modalities and equipment.
• Provide consultation to other health care professionals.
• Communicate with physicians and other health care professionals about changes in needs.
• Attend and participate in case conferences to share knowledge and improve case management for special populations.
• Participate in quarterly peer reviews with documentation.

Responsibilities for Physical Therapist (PT) includes:

• Assessing the client needs on admission for service as appropriate.


• Implement a medical plan of care and revising as necessary.


• Initiating appropriate preventive therapeutic and rehabilitative techniques.


• Educating the client and family regarding treatment modalities and use of equipment and devices.


• Providing consultation to other health care professionals.


• Communicate with the physician and other health care professionals regarding changes in the client’s needs.


• Attends and participates in case conferences, as indicated, to share and/or gain knowledge, especially when services are provided to special populations. Such knowledge supports clinical application of physical therapy in special populations to recognize symptoms and improve case management techniques specific to these populations.


• Participates in quarterly peer review that is documented and signed by both parties
