Job summary

Trinity Health Mid-Atlantic is seeking a licensed Physical Therapist in Muskegon, MI to evaluate patients and provide autonomous PT services within the scope of practice. You will establish goals with patients and families and document according to standards while collaborating with an interdisciplinary team.

The role requires a Doctorate in Physical Therapy (or equivalent), FSBPT-certified bachelor’s, and Michigan licensure or eligibility, plus BLS.

Qualifications

• Must be a graduate of a Doctorate in Physical Therapy or equivalent program accredited by the APTA.
• Bachelor's in Physical Therapy with FSBPT certification is required.
• State of Michigan Physical Therapist license or eligibility before hire date.
• American Heart Association BLS or equivalent.

Responsibilities

• Provide treatment within scope of license and as required by law/regulation.
• Evaluate and diagnose patient conditions.
• Establish treatment goals with input from patient and family.
• Document per internal/external timeliness and reimbursement guidelines.
• Work as part of an interdisciplinary, patient-centered care team.

Employment Type: Full time

Schedule: 40 Hours per week, Full-Time Day Shift

• 8:00am - 4:30pm Monday - Thursday
• 9:00am-5:30pm Friday
• 8:00am - 4:30pm for Weekend Shifts

Weekend Coverage: Colleague will work one weekend (Saturday and Sunday) every four weeks

Holiday Coverage: Colleague will work two holidays per calendar year

This position is based in the House Rehab Services department in Muskegon, MI and does provide physical therapy services on an as-needed basis at Shelby Hospital in Shelby, MI.

Position Purpose

The physical therapist will assume autonomous responsibility for evaluating patient’s need for physical therapy.

What the Physical Therapist (PT) Will Do

• Providing treatment and services within the scope of their license, certification or registration and as required by law and regulation.
• Evaluation and clinical diagnosis of patient conditions.
• Establish treatment goals utilizing input from patient and their family as appropriate.
• Documentation remains in accordance with established internal/external timeliness standards, insurance standards, and reimbursement guidelines.
• Participates as a part of an interdisciplinary and patient-centered care team which includes the verbal and written exchange of information with other healthcare team members as it pertains to integrate a physical therapy treatment plan.

Minimum Qualifications

• Must be a graduate of a Doctorate's or previously equivalent professional Physical Therapy training program accredited by the American Physical Therapy Association.
• Required: Bachelor's in Physical Therapy FSBPT Certification.
• State of Michigan Physical Therapist License or eligible to obtain licensure prior to hire date.
• American Heart Association BLS or equivalent.
