Job summary

LiveWell Homecare Agency, LLC seeks a licensed Physical Therapist to provide in-facility services under the Director of Rehab, assessing patient needs and creating personalized recovery plans to restore mobility.

You will conduct evaluations, deliver interventions, educate patients and caregivers, and collaborate with colleagues to ensure high-quality rehab services.

Qualifications

• Valid PT license in applicable state(s) or proof of eligibility.
• Master's degree in Physical Therapy or related field, or equivalent experience.
• Minimum of 5 years of progressive PT experience if holding a Bachelor's degree.
• Ability to adapt to diverse patient needs and work in various settings.
• Strong collaboration, communication, and teaching skills.

Role Overview:

Responsible for providing comprehensive physical therapy services in an in-facility setting, reporting to the Director of Rehab. The Physical Therapist assesses patient needs, develops personalized recovery plans, and delivers therapy to restore and enhance physical abilities. They serve as both healer and educator, training patients and caregivers to promote independence and mobility.
