Job summary

The mission of this role

Deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on.

Objectives

  • Carry a full, high-quality caseload: ramp to ~28 productivity points/week at target visit quality.
  • Own your patients\' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality.
  • Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent.
  • Set the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows.

What you\'ll own

  • Own the caseload as a case manager: you\u2019re accountable for the patient\u2019s therapy plan, progression, and outcomes 4 not just individual visits.
  • Complete comprehensive in-home assessments and deliver skilled PT visits.
  • Build and execute a clear plan of care: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention.
  • Set measurable goals, track objective progress, and adjust treatment quickly when patients stall.
  • Drive multidisciplinary coordination: communicate changes in condition, align on orders, and keep the care team moving.
  • Manage treatment and outcomes directly, including PTA supervision where appropriate.
  • Coach patients and caregivers with practical home programs that actually get done.
  • Document promptly and thoroughly in Homecare Homebase (HCHB) 1 defensible, efficient, and consistent.

Must-haves

  • Active Michigan PT license (in good standing)
  • Strong clinical judgment and comfort working independently in the field
  • Comfortable escalating changes in condition to RN/MD and documenting rationale clearly
  • Strong communication and documentation
  • Reliable follow-through on orders, frequencies, and patient goals
  • Current driver\u2019s license and comfort with daily travel across assigned territory
  • Able to carry/transport basic therapy equipment as needed
  • Competency in core home health PT scope
  • Safety-first mobility work: transfers, gait training, balance/fall prevention, assistive device recommendations
  • Strengthening, functional mobility, and home safety assessment/modification education
  • Patient/caregiver coaching with practical home programs

Nice-to-haves

  • Prior home health experience
  • Homecare Homebase (HCHB) experience (or similar home health EMR)
  • Deeper clinical experience in high-signal PT populations
  • Experience supervising/mentoring PTAs and collaborating on care plans
  • Strong organization and time management

Our guarantee

  • Full-time target: ~28 productivity points/week; clinicians earn more for exceeding it.
  • Guaranteed floor: ~24 points/week for the first 90 days - if your caseload hasn\u2019t filled, you\u2019re still paid the floor.
