Synchrony Rehab in Ohio is seeking a dedicated Physical Therapist to deliver a full range of PT services, including assessment, treatment planning and interventions in an interdisciplinary setting.

You will educate patients and families, document progress, and collaborate with the care team; prerequisites include PT licensure and a degree from an accredited program; CPR is preferred.

Qualifications

• Must have a current, valid state PT licensure or certification.
• Degree in Physical Therapy from an accredited program.
• 0-1 years of experience in physical therapy (preferred).
• CPR certification is preferred.

Responsabilités

• Conduct thorough assessments to evaluate patients' physical conditions and rehabilitation needs.
• Develop and implement personalized treatment plans with exercises, manual therapy and modalities.
• Educate patients and families on exercise techniques, posture, body mechanics and injury prevention.
• Maintain accurate documentation of patient progress, plans and outcomes in compliance with policies.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.

Job Summary

The Physical Therapist (PT) is responsible for providing a full range of physical therapy services including assessment, treatment planning and therapeutic interventions in an interdisciplinary environment consistent with the position's qualifications, professional practices and ethical standards.

Licenses and Certifications

• Conduct thorough assessments to evaluate patients' physical conditions, functional limitations, and rehabilitation needs.


• Develop and implement personalized treatment plans, including therapeutic exercises, manual therapy techniques, and modalities to improve patients' physical function.


• Educate patients and their families on exercise techniques, posture, body mechanics, and strategies to manage their condition effectively.


• Maintain accurate and up-to-date documentation of patient progress, treatment plans, and outcomes in compliance with facility policies and regulatory requirements.


• Collaborate with interdisciplinary team to ensure comprehensive patient care.


• Other duties as assigned.
