Creative Health Solutions is looking to hire a PRN Physical Therapist to join their growing team!

We are seeking a skilled and passionate Physical Therapist to join our team at Creative Health Solutions. The ideal candidate will work closely with a team of healthcare professionals to develop and administer therapy programs aimed at improving patients' physical, mental, and emotional well-being.

Responsibilities

As a Physical Therapist, you will assist in:

1. Reviewing available patient information and evaluating the impact of this information on the assessment and treatment process.
2. Conducting and documenting a thorough functional assessment of the patient utilizing standardized procedures, including home/vocational and equipment needs.
3. Developing and implementing an individualized treatment plan based on patient needs and containing achievable functional goals.
4. Revising treatment plans, as needed, throughout the treatment process.
5. Completing documentation in accordance with applicable federal and state laws and applicable standards and administrative and clinical requirements.
6. Providing supervision to Assistants in accordance with applicable State Practice Act guidelines.
7. Orienting patients to program services and treatment procedures appropriate to their needs, and informing patients of any potential risk during any procedure.
8. Communicating with nursing staff, patients, and families to ensure effective care delivery.
9. Establishing and instructing patients and family/other caregivers in exercises and therapeutic procedures to be continued following discharge.
10. Assisting in the referral process when a patient requires additional services following discharge and maintaining current knowledge of community resources.

Qualifications

Minimum Qualifications/Requirements:

1. Master of Science degree in Physical Therapy or Bachelor of Science degree in Physical Therapy and five years of direct practice experience.
2. Current license or eligible for license under statutes in the state of practice.
3. Demonstrates competency in applying the principles, methods, materials, and equipment used in Physical Therapy to the patient population served by this facility.
4. Excellent oral and written communication and interpersonal skills.
5. Adequate computer skills in order to complete required job duties and computerized documentation.
6. Demonstrate ability to assess patient needs and develop and implement a comprehensive plan of care.
7. Strong organizational skills in order to effectively maintain multiple projects simultaneously.
8. Requires creativity as well as the ability to work independently.
