Job summary

Interim HealthCare Inc. is seeking a Home Health Physical Therapist to provide one-on-one care in patients' homes across Manchester, NH and surrounding communities. You will evaluate and plan individualized treatments, help patients regain independence, and collaborate with an interdisciplinary team to coordinate care.

The role offers autonomy, flexible scheduling, continuing education opportunities, and a supportive leadership team within a trusted home health organization.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Active New Hampshire Physical Therapist license.
• Minimum of one year of PT experience (home health experience preferred).
• Current CPR certification.
• Knowledge of state and federal home health regulations.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Restore strength, mobility, balance, and functional independence.
• Provide hands-on therapy and therapeutic exercise.
• Monitor progress and adjust care plans as needed.
• Collaborate with nurses, occupational therapists, speech therapists, home health aides, and other clinicians to coordinate care.
• Educate patients and family members on exercises, safety strategies, and ongoing recovery.
• Complete accurate and timely clinical documentation.
