Job summary

HealthPro Heritage at Home is seeking a PRN Physical Therapist to provide skilled PT services in patients' homes across Portage Lakes, OH and surrounding area. You will evaluate patient needs, develop individualized treatment plans, and help patients regain independence with the autonomy that home health offers.

Ideal candidates hold an Ohio license (or eligibility), enjoy one-on-one patient care, and can work independently with reliable transportation.

Qualifications

• Education: Degree in Physical Therapy from an accredited institution.
• Licensure: Valid state licensure as a Physical Therapist, or license eligible.
• Excellent communication and interpersonal skills with a patient-centered approach.
• Ability to lift and assist patients; perform physical tasks required for therapy.

Responsibilities

• Patient Assessment: Evaluate patients' physical conditions, mobility, strength, and flexibility.
• Treatment and Education: Provide exercises, manual therapy, and educate patients and families on home routines.
• Documentation: Maintain accurate records of evaluations, progress, and treatment plans.
• Collaboration: Work with healthcare providers and participate in multidisciplinary team meetings as needed.

Responsibilities

• Patient Assessment: Evaluate patients' physical conditions, including mobility, strength, and flexibility.
• Treatment and Education: Provide therapeutic exercises, manual therapy, and other treatments to enhance functionality and alleviate pain. Educate patients and families on exercises, posture, and body mechanics to aid recovery, prevent injuries, and offer guidance on home routines and lifestyle changes.
• Documentation: Maintain accurate records of evaluations, progress, and treatment plans, ensuring compliance with legal and ethical standards.
• Collaboration: Work with healthcare providers to ensure effective patient care and participate in multidisciplinary team meetings as needed.
