Job summary

Vasona Creek Healthcare is seeking a dedicated Physical Therapist (PT) to join our short-term rehabilitation team in the Los Gatos/South Bay area. You will perform comprehensive evaluations, develop individualized treatment plans, and deliver skilled therapy in a state-of-the-art setting.

Join a multidisciplinary team of PTs, PTAs, OTs, and SLPs; work with advanced rehabilitation technology and evidence-based care to help residents regain function and independence while ensuring safe discharge

Qualifications

• Perform comprehensive physical therapy evaluations and develop individualized, evidence-based treatment plans.
• Provide skilled therapy services to short-term rehabilitation and long-term care residents.
• Treat patients recovering from joint replacements, cardiac events, strokes, and other medical conditions.
• Utilize specialized rehabilitation programs, including Acute Neuro Rehabilitation and Pain Management.
• Monitor patient progress and modify treatment plans as appropriate.
• Collaborate with PTAs, OTs, SLPs, nursing, and other members of the interdisciplinary team.
• Educate patients and families regarding treatment plans, exercises, mobility, and strategies for continued progress.
• Promote safe mobility, functional independence, and successful discharge planning.
• Maintain accurate and timely clinical documentation.
• Advocate for patients and support their goals throughout the rehabilitation process.
