Job summary

Trinity Health At Home is seeking a licensed Physical Therapist to deliver in-home care in Michigan. This full-time, day-shift role involves assessing, planning, and implementing rehabilitation programs for patients in their homes, and collaborating with a multidisciplinary team.

You will educate patients and caregivers, maintain documentation, and participate in quality initiatives. Home health experience is preferred, with a driver’s license and reliable transportation required.

Qualifications

• Bachelor’s degree in Physical Therapy from an APTA-accredited program.
• Current state license to practice as a Physical Therapist.
• Minimum of one year of experience or completion of a 6–8 week preceptorship.
• Valid driver’s license and reliable transportation.
• Home health experience preferred.

Responsibilities

• Provide professional physical therapy services as prescribed by physicians, following state practice acts and organizational policies.
• Assess patient conditions, develop individualized treatment plans, and revise care plans as needed.
• Implement therapeutic and rehabilitative procedures, including manual therapy, exercises, and physical agent modalities.
