Job summary

Proforma Health Inc. is seeking a Physical Therapist in Lancaster, PA to assess, plan, and deliver personalized rehabilitation programs.

You will work with a rehab team to improve patients’ strength, mobility, and overall function, while educating patients and caregivers on home exercise programs. Ideal candidates are graduates of an accredited PT program with an active PT license and CPR certification, and have experience in rehab settings.

Qualifications

• Graduate of an accredited physical therapy program.
• Current PT license required; rehab experience preferred.
• CPR certification is mandatory.

Responsibilities

• Conduct assessments and develop personalized treatment plans.
• Provide direct patient care to improve strength, mobility, and function.
• Educate patients and caregivers on home exercise programs.
• Accurately document treatment sessions and patient outcomes.
• Collaborate with rehabilitation teams and assist in discharge planning.
• Deliver specialized rehabilitation care for patients recovering from neurological, orthopedic, or traumatic injuries.
